Students Requiring Special Diets
Students who require modifications to their school meals for a diagnosed medical condition (ex. allergy, diabetes, celiac disease, etc...) must have a completed Diet Order on file at the School Nutrition Office. These requests must be completed on the Medical Statement for Students with Special Nutritional Needs for School Meals (Diet Order) form, signed by a Recognized Medical Authority. Diet Order forms may be obtained from the School Nutrition website, School Nutrition Office, Cafeteria Manager, or School Nurse. These forms should be returned to the School Nurse. The Nutritionist will modify menus and instruct Cafeteria Managers with regard to each individual Diet Order.
